An AI voice agent is a program that answers (and makes) phone calls and talks naturally. It understands what the patient says, replies out loud and gets things done during the call: booking an appointment, moving it, cancelling it or giving information. At a clinic it does the front desk’s phone work, at any hour.

You’ll also see it called an AI voice assistant or an AI phone receptionist. They’re the same thing. At HeyCAi we call ours CAi.

How it handles a call, step by step

This is what a booking call with CAi looks like:

  1. It picks up in under 2 rings, using your clinic’s name.
  2. It listens and works out what the patient wants. “I’d like an appointment this week”, “any slots on Thursday?” and “I need to come in as soon as possible” are the same request.
  3. It checks your diary right then, while it’s talking.
  4. It offers specific slots: “I’ve got Thursday at 10:00 or Friday at 17:15. Which suits you?”
  5. It saves the booking in your clinic software, with no double bookings.
  6. It sends the patient a confirmation on WhatsApp.

When the call ends, it leaves you a summary and a transcript. If the caller is already a patient, it recognises them and replies with context: “Hi Maria, I can see you’re booked in on Thursday. Would you like to move it?”

What’s inside

A voice agent puts five pieces together, and they all need to be fast for the conversation to sound natural:

  • Speech recognition, which turns what the patient says into text.
  • Language understanding, which works out from that text what the patient wants, even when they speak casually (“my back’s gone again”).
  • The dialogue, which decides what to ask or say next: clarify the appointment type, confirm, or hand over to a person.
  • The connection to your diary, to check availability and save the booking.
  • Speech synthesis, which turns the reply into a natural-sounding voice.

If you want more detail, we cover it in what an AI assistant for clinics is.

Compared with a text chatbot, the difference is the channel: the patient doesn’t have to type anything, they just talk. We compare them in AI receptionist vs website chatbot.

It makes calls too

A voice agent doesn’t only answer. CAi can make reminder calls 24 hours and 2 hours before the appointment, with the option to reschedule there and then. It can also ring patients who finished their plan and haven’t been back, to offer them a review appointment.

What it doesn’t do

It doesn’t diagnose, assess urgency or give clinical advice: that’s for your team to decide. And if something falls outside what it knows, it puts the call through to you or an on-call number, or takes a message and alerts your team on WhatsApp. You choose.

If you’d like to hear one before reading on, try it with your clinic: enter your clinic’s website and your mobile, and CAi rings you.

Frequently asked questions

Is an AI voice agent the same as an AI voice assistant?

Yes. They’re two names for the same thing: a program that answers the phone by talking to the patient and gets things done during the call, such as booking an appointment. If it also answers WhatsApp and manages the diary, it’s usually called an AI receptionist.

Do I have to change my phone number?

No. Keep your number: calls go to CAi when you can’t answer, out of hours or always. You choose.

Which languages does it speak?

It answers in Spanish and English. It picks up the language from the patient’s first few words and carries on in that language.

What happens if it doesn’t know the answer?

It doesn’t make one up. If something falls outside what it knows, it puts the call through to you or an on-call number, or takes a message and alerts your team on WhatsApp. You choose.

What if the system goes down?

Calls go to a backup number your clinic chooses, so no patient is left with the phone ringing out.
